    摘要:目的 探讨髋离断截肢者穿戴假肢的步态特征,并分析其与正常步态产生差异的原因,辅助临床的诊断评估,为新型髋假肢的机械结构和控制系统设计提供实验依据与理论分析。方法 通过便携式人体运动捕捉设备和足底压力分析系统,采集了5名髋离断截肢者运动学和足底压力信息,并与对照组15例健康志愿者比较,对比截肢者与正常人以及截肢者患侧与健侧之间的步态差异。结果 髋离断截肢者双支撑期占比高于正常步态、患侧和健侧的步长、步长时间、加载响应期、支撑中期、预摆期、摆动期占比与对照组相比有显著性差异(P<0.05)。步态时相对称性指数为(0.60±0.05),与患侧相比,健侧支撑期延长,步长缩短,地面反作用力大于患侧,压力中心轨迹向患侧偏移。结论 髋离断截肢者与正常人的步态具有明显差异,其步行能力弱,步态对称性差,身体重心缺乏连贯性。

    Abstract:

    Abstract: Objective To analyze the gait characteristics of hip amputees, and analyze the reasons for their differences from normal gait, assist clinical diagnosis and evaluation, and provide experimental basis and theoretical analysis for the design of mechanical structure and control system of new hip prostheses. Methods Through the portable human motion capture device and plantar pressure analysis system, the kinematics and plantar pressure information of 5 hip amputees were collected, and compared with 15 healthy volunteers in the control group, the amputees were compared with normal people and amputees. Gait difference between the affected side and the healthy side. Results The gait of hip amputees is significantly different from that of normal people. The proportion of the double support period is higher than that of the normal gait, the step length of the affected side and the uninfected side, step time, loading response period, mid support period, pre-swing period, Compared with the control group, the proportion of the swing period was significantly different (P<0.05). The relative symmetry index of the gait is (0.60±0.05). Compared with the affected side, the support period of the healthy side is extended, the step length is shortened, the ground support reaction force is greater than that of the affected side, and the center of pressure trajectory shifts to the affected side. Conclusion Hip amputees have weak walking ability, poor gait symmetry, lack of continuity in the body's center of gravity.
